This application is an agreement between you and the Music City Animal Rescue and its Board of directors. By entering your name below, you are stating that you have read and completed the form to the best of your knowledge. You understand that the agreement may be terminated at any times by either party for any reason; however, by becoming a volunteer you may be given access to our animals as well as other property (intellectual or physical), and you agree to handle all with care, confidentiality, and consideration. By signing this agreement, you understand that you become a representative of the Music City Animal Rescue and agree to represent in a positive and courteous manner.

Should you work directly with our animals, you acknowledge and knowingly accept all risks associated with exposure and handling of all animals for yourself and anyone associated with you (family, friends, etc.) including serious injury or death. THE VOLUNTEER HEREBY WAIVES, RELEASES, DISCHARGES, HOLD HARMLESS, AND PROMISES TO INDEMNIFY AND NOT TO BRING SUIT AGAINST MUSIC CITY ANIMAL RESCUE, its directors, officers, volunteers, staff, all other agents, and attorney for any of the referenced parties and any other parties acting for or on behalf of the referenced parties from an and all rights and claims which the volunteer has or which may hereafter accrue to the volunteer and form any and all damages which may be sustained by the volunteer directly or indirectly in connection with, or arising out of the volunteer’s exposure to the animals.
